The "Programmer's Guide to PC and PS/2 Video Systems" written by Richard Wilton for Microsoft Press (ISBN 1-55615-103-9). It's absolutely invaluable if you're planning on doing graphics on IBM-PC computers. It should answer most/all of your questions.
