We have been fairly happy with the Flagstaff Engineering products for doing this. Earlier versions of their software were fairly buggy but things seem to have stablized in the last year or so. They used to offer DisplayWriter to ASCII but no longer, they now offer DisplayWriter to DisplayWrite III (PC) RFT format, preserving formatting information as well as the text itself. DW III has an ASCII conversion facility so taking it to ASCII just takes a few more steps. The Flagstaff stuff consists of a replacement floppy disk controller for the PC and an attached 8" (or dual 8") drives, an installable device driver and several conversion programs, menu driven, easy to use, if the all caps displays don't drive you out of your gourd. Fairly expensive when we bought it (three years ago), I don't know what they are asking now, I would guess about $2K for everything.
