[comp.sys.misc] What disk errors cause SCSI error 14 when formatting?

I'm running into a problem formatting a Tandon TM262 MFM disk drive with an
Adaptec ACB-4000 SCSI->MFM controller.  After issuing the normal SCSI Mode
Select and Format Unit, the controller starts formatting the drive.  I can
hear the drive being stepped across the cylinders as formatting proceeds.
After a while (maybe 30 seconds or so) Format Unit returns error status
02 00.  A subsequent Mode Sense returns 14 00 5C A3, which is a SCSI 'Record
Not Found' error according to the controller reference manual.  The manual
elaborates, "Logical block ID not on accessed tracks, but no ID CRC error".

How can this error be returned on a formatting operation, which I expect to
be writing the ID fields it's talking about?  Also, does the 00 5C A3 have any
significance?  The controller manual says it's not a valid block number unless
the first bit of the Mode Sense response is set (i.e., 94 for first byte).
Maybe it has some other relevant meaning here?
