I don't know if it's related or not but thought this might be of help.

I have a 1000SL and when I first got I got a copy of ULTIMA 5 also. The 
game ran fine for a few minutes, then completely hung up (sound 
familliar?). *Everything* else ran fine though, so I took it to Radio 
Shack and completely stumped them with that one. The box for Ultima said 
it worked on 256K, which was convient cuz that's what I had at the time. 
While the machine was in, I had them upgrade the memory to 640K. After 
that the game ran fine. It *appears* the program was trying to address 
memory that wasn't there with 256K. I know you said you have 640K 
installed, so you might just check to see that all the chips are seated 
fully, etc, you may be running into the same thing. That's may best (and 
only) guess.
