Checking my handy HP 150 Tech Ref Manual (Can we say "Worked on the 150 before it was new?), an error 0004 is a memory error, specifically chip U44. Looking at the mezzanine memory card from the top with the roms on your right, it's the 4th row of chips (3rd rom of memory chips) and the 4th column of chips (all memory chips) from the left. I might have an antique mezz board laying around somewhere, but if I do it will be a prototype... I'll check. Otherwise, you can probably desolder the chip and replace it. Good luck! Miles
