[comp.sys.mac] Mini-debugger gone west

I have been having trouble several times trying to escape from
something via the mini-debugger and a typed in ExitToShell.  I finally
decided to go after it this morning:

Setup: Mac plus (upgraded twice from 128K), Apple HD20, Apple 400K
external (yes, I still use it), Jasmine DD50 (this problem predates
the Jasmine); 4.2 / 6.0, lots of INITs.

1. I pulled all the inits and non apple cdevs out of my system folder
and restarted; NG.

2. I reinstalled the System from a locked System Tools 1 disk; NG.

3. I rebooted from the locked S T 1 disk; NG

4. I turned off both hard disks and rebooted form the S T 1 disk; NG

When I press the interrupt button, I get a flicker on the screen
(about one frame worth) and the mouse freezes.  Again, this even
happens with the hard disks powered off and booting from a locked copy
of the Apple System disk.  Any ideas what's wrong or what I should
try?
