[net.wanted] machine reading a questionaire

I have an application requiring a machine-readable questionaire.  Does
the net have any suggestions as to what is the cheapest/easiest way
to go about this?  The application involves some medical data collection
using patient-supplied data.

Some specs:
	The input will be relatively low volume.  That is, only one or
	two questionaires will be entered into the system at a time.

	The questionaire should not be too annoying/complicated/clumsy
	for the patient to fill out.  For instance, those score sheets
	on which you fill in the little ovals with a No. 2 pencil is
	probably not acceptable (remember those things?)

	It should be able to interface with an RS-232 port on a micro.

	Because it will be used with a micro, the reader shouldn't cost
	more than the IBM-PC it will be used with.

I know, an impossible set of restrictions, but I am just surveying what
is available.
