There's a rather annoying bug in Ultima V. After inserting a write-protected 'Player/Underground' disk (which is the save disk) you're told that that's the wrong one and to insert the right disk. All you have to do to damage your disk is removing the write-protect, put it back into the drive and hit any key... Then, when you try to leave Iolo's hut, the game hangs saying 'offsetmap'.
