I had the same problem on my first try. On the second try (luckily the shop had two Ultima V), I found out that this problem is home-made. It occurs when you insert a write-protected player disk and remove the write-protect after you're told that you inserted the wrong disk. On the next write to this disk, the program thrashes the disk and you get 'offset map' when you try to leave Iolo's hut. If you were playing from a backup, no problem.
