Thanks for all the responses pointing me to information about
the GPM macrogenerator.  I include the relevant references:

Chris Strachey "A General Purpose Macrogenerator", Computer
Journal, October 1965 pp 225-241 (includes the implementation!)

A.J. Cole "Macro Processors", Cambridge University Press 1981
(note that this is the second edition)

Peter Wegner "Programming Languages, Information Structures,
and Machine Organization", McGraw-Hill 1968

P.J. Brown "Macro Processors and Techniques for Portable
Software", J Wiley & Sons 1974

Also mentioned were the macro languages stage2, ML/I and TRAC,
most of which are mentioned in the above references.
