for tons of technical details about IBM-standard video adaptors,
get a copy of:
title: Programmer's Guide to PC and PS/2 Video Systems
author: Richard Wilton
publisher: Microsoft Press
it's a big, heavy paperback that's very complete and contains
lots of assembly-language examples.
it also contains a mail-in card for a disk containing all the code
that's printed in the text.
i used it with success in programming the EGA recently.
unfortunately, the organization and indexing are pretty terrible,
and many of the really arcane details must be gleaned from the
assembly-language examples.
nevertheless, if you look hard enough, you'll find all you want;
and i don't know of any other reference that's nearly as complete.
in the land of the blind one eye is king.
