I called Microsoft yesterday asking about just this problem. It seems
that Microsoft themselves supports only 8 colors in Windows. However,
the standard they define allows for more. At least for EGA and VGA 
some company sells a 16 color driver to go with a program they sell.
It wasn't clear if they sell it separately; I didn't write down the
address since for my application users can't afford to pay for
extra drivers-  I think it is silly that Microsoft doesn't use all
the colors available, but that's the way they are. In any case,
apparently money can solve this problem.
