[comp.sys.ibm.pc] Mark Sense, or other survey input device

We are looking for a way to gather survey information into a IBM PC type 
environment.  We want to keep the input process simple for those filling
out the survey, as well as keeping the task of processing the replies, painless.

We think that an image scanner would be a programming nightmare in terms of
selecting only the user's input information. And it seems that since a number
of 'standardized test' use a simple input format, that an easy-to-use,
easy-to-program device must be out there somewhere.

Any suggestions??
