[comp.sys.apple] Quantum/AppleLink PE Address

Can somebody send me the full mailing address to Quantum/Applelink.

In March at Applefest in Boston, I filled out a card that was supposodly
going to entitle me to purchase the Applelink Personal Edition version
for a special discount (and in advance no less...).  They told me they
were going to send out the forms in 1-2 weeks.  Its been 4+ months
(sorry, I meant to say May...) and I still havent heard from them.
