You're not going to like this answer (I didn't either), but try moving the
Grappler to slot 7 and your CMS interface card to slot 6.

I had the same problems recently when I installed a CMS 20MB drive on a //e
with the Apple parallel printer card.  The system locked up completely unless
the printer card was removed, and CMS "Technical Support" (cough, choke) was
absolutely no help whatever.  They swore up and down that their drive was
fine, and that the problem was due to an "antique" interface card.

When the system still wouldn't work after trying two other printer cards I
came very close to returning the drive for a refund.  Then, out of
desperation, I tried moving the printer card to slot 7 and the CMS card to
slot 6.  Voila!  It worked!  It's kludgey, but it gets the job done -- as
long as you're using software that can re-assign the printer to slot 7.

Another word of advice: the latest CMS ROM's don't work on the ][+ (despite
what CMS tech support says).  You have to use the older ROM's, which require
the drive to be up-to-speed before powering up the computer.
