[comp.lang.smalltalk] OOPSLA '88 Volunteer Signup Address Wanted

I was at the Orlando conferance and aquired some of the volunteer 
signup sheets.  I want to send these sheets in to the OOPSLA '88
program commitee, but there isn't any address on them.  Who knows
where I am supposed to send this stuff ?
