[misc.wanted] Looking for tractor feed labels and index cards...

I'm looking for tractor feed diskette and address labels, and tractor
feed index cards, all suitable for using with an Imagewriter printer.
Does anyone know of any companies that produce such things, and if so
could you mail me there address and phone number?  Thanx much!
