I did that about 1 1/2 years ago.  I came up with a very nice result.  Maybe
one day I'll post it.  However, if you want to make your own, a good way to
start is to use a thick black line around the perimeter as well as on the
folds.  If you print your labels on heavy paper (I use botany paper), in
high-quality mode, you will find that the paper folds perfectly every time.
