This is a second request; does anyone have a Fortran cross-reference program
(pd or otherwise) which may be had? I would appreciate hearing any leads.

Have you seen the "xref" program?  It is a pretty general cross 
reference tool for 4.XX unix, and works with documents, C, fortran, etc.
You load it with tables of keywords, stopwords, etc.
The name of the author escapes me.
