[net.music.classical] Stage etiquette

Does anyone know of any books on stage etiquette?  I'm looking for
something on the basics of stage presence (bowing, when to acknowledge
accompanists, when to accept first bows, etc.), etiquette in the 
general area of performance (never practice someone else's solo, etc.),
and the like.  

As far as I've been able to find, no such volume exists.  Journal 
articles and such would also be appreciated.  

Also, I would be interested in any comments anyone might have in this
area, including contributions anyone could make to my collection of 
"behavior codes."  I could post my compilation to the net at a later
date if anyone is interested.

I know of no book about this but let me contribute a rule
from my experience as a much-abused oboe player who had to
sit directly in front of the trombones:

	Look carefully before emptying your spit valves.

(Then again, maybe they did!)
