You might find something interesting in the first few pages of the Starlog
Doctor Who Super Special (I *think* that's the name...).  They have bits
of each one of the 5 various opening sequences as still photos.

Apparently the theme has never changed.  It's been perfomed differently (e.g.
the current electronic version vs. the one that you might have heard at the
end of the Five Doctors) but I think the music is all the same.
Also, I remember seeing a "tunnel-effect" opening for a Pertwee episode,
The Time Warrior; the only differences were that the face was Pertwee's
(naturally) and that the tunnel's color was GREEN!  (Has anyone else
ever seen this?)

I believe this bridge was used only in the end-of-season episodes when the
series was 6 episodes long and the credits on the last episode were longer.
Then the bridge that gets into the Davison credits is used.

Yes... I used to do terrible things with the music at my old high school.
I used to start up the game in our computer room on 8 Apples simultaneously
to irritate the Math teacher...

I apologise for the fuss, folks.  I guess I was wrong-  It just seemed
that there was overriding evidence.  The reason I thought those two
questioned episodes would not be shown was:
a) The adventures in question were available in a motley Colour/BW mix
which might be unshowable.
b) The episodes in question were supposedly "not BBC broadcast quality."

Oh well, onto the theme.
Someone else is going to answer this question, I know, so I'll just have
to answer it more thoroughly.

An Unearthly Child-  The Original theme and credits are shown, using howlaround
and, significantly, without the Doctor's face.  Episodes have individual
titles.  No graphics used for story ending credits, just black w/ lettering.
Savages- First story with no individual titles.
War Machines- Title written in computer lettering.
Smugglers- "Story so far" on simulated old lettering replaces for one
story the traditional prequel.
Power of the Daleks- First Patrick Troughton story- Same faceless credits.
Macra Terror- A new graphics are devised for the 2nd Doctor, featuring
his face.  Ending graphics are also devised, but used only this story.
Faceless ones- Formula of new opening graphics with old black ending credits.
Spearhead from Space-  "spangles" added to original theme.  Music
changed so as to lead more smoothly into adventure, with color graphics
featuring orange tongues of flame around Doctor's face. (Note: For this
adventure, the words "Spearhead from Space" approach the screen instead
of just appearing.
Silurians- Only adventure to have "Doctor Who" in title.  Title reads,
"Doctor Who and the Silurians."
Ambassadors of Death- Prequel is edited into theme.  First adventure to
have now traditional whizzing sound at the end of episodes (And in the
starting theme)
Inferno- Another of many stories to have a credits sequence featuring
the story name, etc.
Terror of the Autons- Starting Music changed back to original format for
two stories.
Three Doctors- A completely new, electronic theme is devised.  The theme
is used on trailers for the "Three Doctors" and a copy of "Carnival of
Monsters" with it goes out to Australia before Barry Letts decides he
doesn't like the new theme, and opts to keep the old one.
Time Warrior- New Graphics start this story, uneconomical since Pertwee
only remains on season.  These are completely animated, by Bernard Lodge.
Baker Era- "Bridge" disappears from ending theme, except on some
six-parters where the credits are so long as to necessitate the bridge.
New animated credits, the only ones to feature the Tardis.
Leisure Hive- New Baker credits, animated by Sid Sutton.  New
electronic theme arranged by Peter Howell.  New "Outer space noise"
leads into adventure.  Electronic music used entirely for the first time
since "Terror of the Autons"
Logopolis- Ending graphics do not feature the Doctor's face.  Instead, a
box-shaped graphic is used.
Castrovalva- New Davison graphics with "Venetian blind" effect.
Five Doctors- Does not feature "Outer space noise", but leads into start
of "bridge," then leading into adventure, rather along the lines of the
Hartnell and Troughton adventures.
Twin Dilemma- New graphics for Colin Baker's Doctor.  Are much more
box-shaped, along the lines of ending credits for "Logopolis"

And that's about all.
