		     COMPUTER MAIL QUESTIONNAIRE

This questionnaire is designed to collect information on your experiences in
using computer mail.  While we hope you will assist us in this research
project, completion of the questionnaire is voluntary.  All necessary steps
will be taken to avoid identification of individual sources.
