Has anoyone heard about a version of Prolog implemented
in Forth?  Any details on how it was done, where to get it, etc.?

No, I haven't, but I have seen a version in Lisp claimed to be the
shortest Prolog interpreter in the world. Two pages, if I remember
correctly. In the book "Implementations of Prolog". Maybe you can
adapt that?
