The solution presented stated that East most likely has the AH and West
has the JH. So cover the QH with the KH and ruff the AH. Play for the
QD to drop either doubleton from East or squeezed out of West.
  
Personally, I wouldn't place too much money on the outcome of this line
of play. However there is a line of play which guarantees the contract with
the very reasonable assumptions that East has the AH and West the JH. It 
does not matter who has the QD or how many times its protected. Keep trying!

On the penultimate spade, dummy throws a heart.  Both east and west must
save both hearts to prevent the honor from being ruffed out, setting up
the ten.  Now south runs the diamonds, dropping the queen, and enters his
hand by ruffing a heart, and cashes the diamond T.

Very pretty hand.
