Win the KH and play AK of diamonds. If the TD drops you have two entries
to dummy to finesse spades twice. Otherwise continue playing diamonds
pitching a club and spade from dummy. The situation before the opponents
pitch on the last diamond is:

First East--
 1)pitching a club sets up the J
 2)pitching a spade allows the spade suit to come in with one finesse
 3)so must pitch a heart
And West--
 1)pitching a club results in an end play after cashing heart and club
   winners and taking a spade hook.
 2)pitching a spade allows declarer to lead low from hand forcing two
   spade winners(drop or using entry for finesse)
 3)so must also pitch a heart
  
Now declarer has two entries to dummy(overtaking QH) to take the spade
finesse twice for the contract.
