Take the King of Hearts and 4 Diamond tricks.  After 5 tricks:

Lead the 3 of Diamonds.  Sluff the 5 of Spades from the dummy.

West *MUST* play a Heart.  a) If West plays a Club, South takes the Ace
and King of Clubs, the Queen and Ace of Hearts and throws in West with
Spades, with only Spades to return for a winning finesse.  b) If West
plays a Spade, South plays the Jack of Spades.  If West wins with the
Queen, South can win the return and go to the Ace of Hearts to finesse
the Spade.  If West ducks the Jack of Spades, East must win with the King.
With any return, South can drop the Queen of Spades and score the 9.

East *MUST* also play a Heart.  a) If East plays a Spade, cross to the
Ace of Hearts, lead the 10 of Spades, cover the King if played, else let
it ride.  Win the return, play the Ace and Jack of Spades.  b) If East
plays a Club, take the Ace and King of Clubs, cross to the Ace of Hearts,
take the Jack of Clubs and the Ace of Spades.

Since both West and East must sluff a Heart, South has *TWO* entries to
the dummy to make 2 Spade finesses, one of which will win and make the
12th trick.
