[net.micro.pc] Op-Scan Reader Wanted

Does anyone know of a relativley inexpensive, i.e. private person
affordable, op-scan reader?  Op-scans being those "fill-in-the-dot"
computer read surveys that can interface to a PC?  Or of any companies
that make said hardware?
