[net.micro.pc] "Type 15" PCAT Drives

I intend to replace the existing 30 meg hard disks on a couple of PCATs with
80 megabyte Maxtor XT-1105 drives.  Now I have the article "Adding a Hard Disk
to Your PC AT" from the Fall 1985 Byte special issue, and have also read the
BIOS listings, but I just don't see how one can really use Type 15 at all.

Would someone with experience please answer these questions:

1) Is it possible to boot from a type 15 drive?  How do you do it?  It seems
   that either the system would have to boot saying that it is another drive
   type and then supply the drive table, or one would have to boot off of a
   floppy all the time.

2) How can you format one of these drives?

3) While Type 15 is implied as being "user definable", it appears that it
   actually causes an extended table of drives to be used (my drive is
   missing from the table).  It appears that type 15 should not be used!
   The Byte article is completely wrong on this point.

4) The Byte article implies that special software is needed to have more than
   one active partition in a hard disk >32 megabytes.  Why can't FDISK be
   used.
