I agree with !porges - James Tiberius Kirk. as stated in "Court Martial".

Check the print media novel of Star Trek The Motion Picture. Kirk himself gives
his first, middle and last names (and the reasons therfore). Since the book was
written by Roddenberry, I take this to be reasonably accurate information.
