Here is the most recent and most complete compilation so far.

The following persons have also appeared in more than one
episode, some of them as crew members and some not.

Bobby Bass was a crewman in This Side of Paradise, a Klingon in
Errand Of Mercy, and a guard(one of Khan's) in Space Seed.
Frank Vinci was one of the Vulcans in the procession in Amok Time,
and an Eminiar guard in A Taste of Armageddon.
Paul Baxley was the Black Knight in Shore Leave, the patrol leader
in A Private Little War, a trooper in Patterns Of Force, and the
security chief in Assignment: Earth.   
All of these latter individuals also did stunt work in addition to
their bit roles.

Although not an answer to the original question, Majel Barrett has
portrayed two different characters(both crew)-she portrayed Number One
in "The Menagerie" parts I and II (she used the name M. Leigh Hudec
at the time) and Nurse Chapel in many other episodes.

If we include the Star Trek Movies, then Mark Lenard also
portrayed a Klingon in Star Trek The Motion Picture.
