This one I saw with my own beady little eyes: In the late '60's, the University of Stockholm and the Swedish National Defense Research Institute replaced an IBM 7094 with a brand-new 360/75. Every afternoon, between 3:30 and 3:45, it would crash. The crashes were so regular that we used to take our coffee breaks at that time. The operators and service technicians would gather around the console to watch for the crash. Eventually, they noticed that the janitor was carefully vacuuming the carpet in the computer room at the same time. The fix proved simple -- in retrospect. Martin Minow decvax!minow
