I don't know if this is the same one you heard about, but this happened in
a minor league game:
			
			The bases were loaded.  The batter hit a slow roller
to third base.  The runner on third ran into the coaches box, and the coach ran
to the dugout.  It appeared to the fielder that everyone was safe, so he didn't
attempt to make a play on any of the runners.  Therefore, the player standing inthe coaches box could not be called out for being out of the baseline.  

Apparently no one in the field noticed this, and the next batter hit a 5-run 
homer.
