I have been suprised at the large numbers of sites installing RFS. As
a favor to me, would the people who actually installed RFS (or next of
kin) fill out and send the following note to me so that I can see how
and where it is being used. The form is intended to be very terse
(minimum work on your part, and mine). Please don't spend any great
amount of time on this or try to qualify your answers... just fill it
out with whatever info is in your head. Thanks.
