Along with phone surveys, the Nielsen "rating service" periodically mails booklets to viewers who agree to record the shows watched on all the sets in the house. I don't know how the recorders are selected, but, back in my teenage days, my family once filled out a Nielsen book. After we sent it back, they sent us a thank you note and two quarters (!) for our trouble. Perhaps this was done during sweeps months (February and November, I think) to compile local ratings.
