I can answer half of the question: The actual title of the M*A*S*H theme is "Suicide Is Painless." It was played (and sung!) in the original M*A*S*H movie. The movie credits mentioned the title (which I remembered) and also the songwriter (which I don't).

I don't remember all the lyrics to the original, but I'm pretty
confident the chorus went like this:

Why do I recognize the name Loudon Wainwright III? What else did he do?

Loudon Wainright III played a dentist for one season of MASH.
I believe it was in the third year of the show.
I can't remember his MASH name. I think he bunked in the swamp
with Hawkeye, Trapper and Frank after Spearchucker had left.

Loudon Wainwright III had a large number of bit parts on MASH
One was as a singing soldier with a guitar who was anonymous
and sang songs about the theme of the episode.
One example was when Hawkeye was trying to get to Tokyo and the
war was getting in the way. Occassionally the scene would shoot
to Wainwright singing:
Oohh Tokyo.
They got some saki and sashimi
and some clean sheets ooh komono, komono.
