One of the best screwups I ever saw on TV news was that during the local TV weather for a while they were going crazy with the chroma key and they had a weather map constructed and they would "pull" other tables (usually the 5 day forcast) out of the middle of the map. One day the weather man pulled out the five day forcast and there was the anchorwoman on that screen. "Whoops that's Rudy Miller" and pushed it back into the map. -Ron

Lauren mentioning local TV station screw ups and KABC-TV in Los Angeles in particular reminded me of a screw up that occurred some years ago on that station. Elton Rule, who is now the head of ABC, was the station manager at KABC-TV. He taped a station editorial for the nightly news and blew his lines about half way through. He shouted some profanities (several of the 7 forfidden words) and they retaped the editorial. The only problem was that they didn't erase the first taping and when the tape was queued up they started from the first try. Needless to say heads rolled.
