     Sho Nuff asked about quaffing more han one potion at a time.
     I do not allow it, the DMG has a table about side effects when
     two potions are mixed internally.  Effects certainly aren't
     cumulative.  I also wouldn't let a character wish for super-heroism
     potions.  I allow wishes for weaker potions, like healing, sweet water,
     even giant strength, and with a wish a PC can go up 1 (and only 1)
     level permenently.  I guess I would allow a wish for a temporary
     increase in level, +5 levels, and since wishes are so powerful,
     I would allow their effects to be cumulative, ie., 2 wishes = 
     +10 levels for a round or two.  That would still mean the Paladin
     w/ Holy Sword could be high level, but getting the necessary
     #attacks in to kill the big O is still a problem.  Wishes again
     could make one attack faster, with enough wishes, I suppose it
     would be possible to get enough attacks.  But why not stipulate
     that this Holy Sword has a special purpose to destroy demons of
     the Abyss, with a powerful special ability, like disintegrate to
     help?

     The solution with a lvl 1 thief with two spheres of annihilation
     is weaker.  How is he supposed to summon/control these?  He
     (or she) is no MU, I would say they would move for the rogue
     and maybe if he was lucky he could gate away before they got to
     him and just after O. entered the room.  I would also give O.
     his magic resistance against the explosion, taking 1/2 damage
     if he fails.  It is a magical effect caused by magical devices.

     Trap the Soul won't work on O.  His soul resides in his soul
     Object, hidden somewhere.  He can be imprisoned though, and
     I think that could be done with a bag of holding.  (At least
     the way I allow my players to use bags.)

     You have a 3 wish wish ring.  Wish your bag of holding to increase
     in size for a couple of rounds, so it's mouth is 10x10.  Next
     wish for the bag to float above the entrance way, mouth open.
     When the big O. enters the room, use your suprise round to wish
     that the bag engulf him.  The next round grab the mouth of the 
     bag, squeeze it shut, and use your amulet of the planes to 
     get to the 7 Heavens.  When you get there, use another wish
     to get the attention of a solar or a couple of planetars and
     give them the bag.  The key to this solution is my rule that
     whatever is in a bag of holding is suspended in time and is in
     no longer in the plane where it was bagged.  Thus the big O.
     wouldn't be able to do anything while caught in the bag.  Even
     if he could he might be cautious about trying to break out, knowing
     that if the bag is cut or penetrated, he will spend the rest of
     eternity in nil-space.  As for his magic resistance, he should
     get a roll against the bag engulfing him.  50%, since the wish
     is 18th level.  Even if he makes the roll he will be partially
     covered and most importantly, his head would be inside.  This
     would negate his psionics and spell casting ability and you 
     could use a wish  to get the rest of him in the bag.  Since the
     above solution doesn't use 5 items, the rest could be more wish
     rings to take care of unforseen happenings.  9 wishes with
     3 rings should be enough to take care of anybody. (of course, how
     your party could be killed while wearing 3 wish rings...)
     Other people play bags of holding differently  of course, it all  
     depends on the DM.  Another method is to put the enlarged bag
     on the floor and wish it completely invisible for a round, then
     let O. step in it after taunting him.
