I think that the major problem with any high-level campaign that gets run is that it quite often (not always, or even usually, but often) turns into a "Let's go trash Hell/Gehenna/Valhalla/whatever!" game, which is utter garbage. In my opinion, the gods/demons/etc. are invincible on their home ground (how else do they hold off the enemies they get?). But I've heard tell of groups gating to the Outer Planes and killing all in sight. Another gripe about mega-level characters is that death soon gets to be a mild nuisance. With the wishes abounding, every character has the ability to keep their Constitution up high enough to survive nearly any death, and get wished alive again. One such case is an acquaintance of mine complaining that her character died four(!) times in one battle and the rest of the party was cutting her out of the treasure. Game balance??? For me, I prefer a low to mid-level (1-7 or so) game. That way, kobolds are deadly for you if enough of them get you, but you still can blast huge numbers of them later on. The best fun is in the *role-playing*, which I find gets lost in the mists of levels.
