Here is a copy of a consent form which I've used for several years. It is adapted from a form our Corp legal department put together for use by training groups et al that make wide use of photos of people in the work setting. I also have a longer and more formal one (copies on request) but it tends to intimidate people more than this. There are no copywrite problems with this! Use it, or modify it to suit your personal requirements. If you are photographing children or minors be sure and get their parents or guardian to sign as it is a legal document. Make sure you have some sort of cross index so that you can associate the proper release with the photos. I use negative material so I reference by roll ID and frame number range. Also remember you don't need a release to take a picture EVER. You only need the release if you expect to use the image for gain; contests, exhibitions, advertising, books,...
