Vision requirements for a Class II medical certificate are:
   
   1. 20/20 vision with/without glasses
   2. 20/100 without glasses.
 
If your vision does not meet these requirements, you may
request a waiver of the requirements by doing the following:
 
   -  Have an ophthalmologist fill out a "Report of Eye
      Evaluation" for your eyes.  The form for this report
      may be obtained from the GADO or your 'flight surgeon'.
      
   -  Request that your doctor send the completed report along
      with a request of waiver and the result of your Class II
      physical to the FAA in Oklahoma.
    
You save time by having your eyes examined before you take
the Class II physical.
  
You may obtain more information on this topic and request
the Report of Eye Examination (and any other forms your
doctor may not have) from the GADO medical department over the

The person at that number explained that the process is
routine.  If your vision problem is limited to myopia and
is correctable to 20/20 you should have no problem with
receiving a waiver.
